If you are transferring to California State University Channel Islands from another institution, you will need to complete a Request for Change of Program or Place of Training form. This form is also available online at http://vabenefits.vba.va.gov/vonapp/main.asp.
In order to receive your benefits, the VA Certifying Official at CSU Channel Islands must certify your enrollment. To ensure timely certification, please submit the following documents to the Certifying Official:
